外國公司中譯名字
(Chinese Name of the company)

• 名稱必須加註國籍。

外國公司之法人資格證明文件
(Photocopy of the company’s certificate of incorporation)

• 由外國公司登記主管機關簽署核發。
• 應附中譯本。

訴訟及非訴訟代表人授權書
(Power of attorney appointing the company’s litigious and non-litigious agent)

• 必要時，主管機關得要求須經經驗(公、認)證。
• 應附中譯本。
• 授權書(POA)應具體訂明代表人在中華民國境內所為法律行為之種類，例如：代表本公司簽訂契約、報價、投標、採購、市場調查及研究等行為。

身分及住居所證明文件
(Photocopy of the identification card of the company’s litigious and non-litigious agent)

• 中華民國國籍→檢送身分證影本。
• 僑外人士，其身份證明文件為下列之一：
  • 護照影本 (加註地址、親筆簽名、及加蓋本人印章)；
  • 其本國政府出具之居住所明文件；
  • 本人出具聲明書（應敘明姓名、國籍、地址），經我國法院認證。

房屋租賃契約書影本
(A photocopy of the original house lease contract)

• 需以(辦事處中文名稱+代表人)名義承租。

房屋使用權同意書
(An agreement issued by the house owner)

• 若向二房東承租，需另行準備由建物所有權人同意之同意書。

房屋稅稅單
(A document to prove payment of the latest house tax)

• 當年內有效。
• 請注意是否完稅。
